摘    要:
目的 探讨不同氧气雾化吸入方法对慢性阻塞性肺疾病急性加重期(AECOPD)的影响.方法 将120例AECOPD患者随机分为持续雾化组、间歇雾化组、先后雾化组各40例.3组患者进行常规治疗和护理的基础上行不同方法氧气雾化吸入,比较3组雾化吸入效果和不良反应.结果 先后雾化组脉率第1次治疗结束1 h较治疗前减慢,治疗结束1 h先后雾化组脉率慢于持续雾化组;间歇雾化组和先后雾化组呼吸频率第1次治疗结束1 h较治疗前均有所减慢,间歇雾化组和先后雾化组第1次治疗结束1 h呼吸频率慢于持续雾化组;3组患者SpO2第1次雾化吸入治疗结束1 h较治疗前均有上升,先后雾化组第1次雾化吸入治疗结束1 h的SpO2高于持续雾化组和间歇雾化组,差异均有统计学意义(P<0.05);3组患者雾化吸入体温、收缩压、舒张压治疗前与治疗结束1 h比较,差异均无统计学意义(P>0.05).先后雾化组雾气湿冷和刺激性呛咳发生率少于持续雾化组和间歇雾化组,且心悸和胸闷气促加重发生率少于持续雾化组,差异均有统计学意义 (P<0.05).结论 先后氧气雾化吸入法能促进AECOPD患者脉率和呼吸频率的稳定,提高患者SpO2,减少不良反应的发生.

Abstract:
Objective To explore the influence of different oxygen atomization inhalation methods on acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 120 AECOPD patients were divided into continuous atomization group,intermittent atomization group and successive atomization group,where there were 40 patients each. Different oxygen atomization inhalation methods were conducted to the patients of three groups based on regular treatment and nursing to compare the atomization inhalation effect and adverse effect of three groups.Results The pulse rate of successive atomization group slowed down 1 hour after the first treatment ended compared with it before treatment.1 hour after the treatment ended,the pulse rate of successive atomization group was slower than continuous atomization group,intermittent atomization group and successive atomization group compared with it 1 hour after the first treatment ended.The respiratory rate slowed down 1 hour after the first treatment ended compared with it before treatment.The respiratory rate of intermittent atomization group and successive atomization group was slower than continuous group 1 hour after the first treatment ended. SpO2 of the patients of three groups rose compared with it of 1 hour after the first atomization inhalation treatment ended. SpO2 of successive atomization group was higher than continuous atomization group and intermittent atomization group 1 hour after the first atomization inhalation treatment ended,and the difference had no statistical significance(P<0.05). Compared with it 1 hour after treatment ended,the difference of atomization inhalation temperature,systolic pressure and diastolic pressure before treatment for patients in three groups had no statistical significance(P>0.05).The mist wet cold and irritating cough occurrence rate of successive atomization group was less than continuous atomization group and intermittent atomization group.The occurrence of palpitation and chest anhelation of successive atomization group was less than continuous atomization group.The difference had statistical significance (P<0.05).Conclusions Successive oxygen atomization inhalation method can promote the pulse rate and respiratory rate of AECOPD patients to be stable,improve SpO2 and reduce the occurrence of adverse effect.
