慢性乙型肝炎血清e系统与HBV DNA的相关性

目的　探讨慢性乙型肝炎患者血清e系统与HBVDNA的相关性。方法　采用ELISA法及荧光定量PCR法 ,检测 60例慢性乙型肝炎 (慢乙肝 )患者乙肝病毒标志 (HBV M)及血清HBVDNA含量 ,并与肝组织炎症活动度进行对比。结果　慢乙肝患者e系统与血清丙氨酸转移酶 (ALT)无显著差异 (P >0 .0 5 ) ;但HBeAg阳性组血清HBVDNA含量显著高于抗 HBe阳性组 (P <0 .0 1 ) ,在各级肝组织炎症活动度组 ,其HBeAg阳性率与抗 HBe阳性率在各组间无显著差异(P >0 .0 5 )。结论　临床上不宜单独凭慢乙肝患者e系统的检测结果作为评判病人是否有HBV的复制的指标 ,而应采用荧光定量PCR法 ,动态观察慢乙肝患者血清HBVDNA水平的变化 ;有条件的地方应尽可能开展肝脏活体组织检查术 ,以正确判断肝组织的病变。

Correlation between the Serum e System and HBV DNA in Patients with Chronic Hepatitis B

Objective To study the correlation between the serum e system and HBV DNA in the patients with chronic hepatitis B. Methods By ELISA and fluori-metric quantilalive PCR the contents of HBV-m and serum HBV DNA in 60 patients with chronic hepatitis B were detected and compared with the inflammatory activity of hepatic tissues. Results There was no marked difference between e system and serum ALT in patients with chronic hepatitis B (P>0.05). The content of serum HBV DNA in the positive group with HBeAg were markedly higher than that in the positive group with anti-HBe(P<0.01). In the group with all kinds of inflammatory activity of hepatic tissues there was no marked difference between the positive rates of HBe Ag and anti-HBe(P>0.05). Conclusion Whether the patients had replication index of HBV or not could not be judged only by the detection results of e system clinically in the patients with chronic hepatitis B, but should be judged also by fluorimetic quantitative PCR and dynamic observation of the change of serum HBV DNA level. To judge the pathologic change of hepatic tissues biopsy of liver should be carried out in the place with good conditions.
