哨兵淋巴结活检预测结肠癌淋巴结转移的价值

目的探讨哨兵淋巴结活检(SLNB)预测结肠癌患者淋巴结转移状态的重要性和可行性。方法使用亚甲蓝对42例结肠癌患者进行SLNB,随后行标准的结肠癌根治术。哨兵淋巴结阴性患者进行连续切片病理检测。结果SL-NB成功率为80.95%,敏感度为97.06%,特异度为100%,预测淋巴结转移状态率为95.45%,假阴性率为4.55%。结论利用亚甲蓝淋巴定位方法进行的SLNB可以准确预测淋巴结转移状态,SLN连续切片、HE染色检测有助于提高SLNB的敏感性。

Sentinel lymph node biopsy in colon cancer;value for predicting the status of lymph node

Objective To investigate the significance of sentinel lymph node biopsy(SLNB) for predicting status of lymph node in patients with colon cancer.Methods Forty-two patients with colon cancer received SLNB guided by methylenc blue staining,and radical operation of colon cancer.Multilevel section was performed in lymph nodes of patients with negative nodes.Results SLN was identified in 34/42 cases(80.95%).The sensitivity of SLNB was 97.06%,the specificity was 100%.Lymph node status was accurately predicted in 21(95.45%) of 22 cases.In 1 of 22(4.55%) cases,the SLN was false-negative.Conclusion Our study indicates that operative lymphatic mapping using a vital dyeing and SLN can accurately predict the lymph node status.Multilevel section and HE dyeing of SLN can improve the sensitivity of SLNB.
