液基薄层细胞学检测联合阴道镜LEEP术在宫颈病变诊治中的应用

目的探讨液基薄层细胞学检测联合阴道镜LEEP术在宫颈病变诊治中的临床应用价值。方法选择在我院2009年9月至2012年2月妇科门诊诊治的患者1026例为研究对象,采用液基薄层细胞学检测联合阴道镜对宫颈病变患者进行筛查,确认宫颈病变的患者采用LEEP术进行治疗。结果本组1026例患者经液基薄层细胞学检测联合阴道镜检查,共查出宫颈病变患者122例,占11.89%,121例患者接受LEEP手术治疗,手术时间2～10min,平均手术时间(4.21±2.05)min;术中出血量2～45mL,平均术中出血量(9.56±6.57)mL;切口愈合时间3～10周,平均(6.52±3.20)周。56例慢性宫颈炎患者半年治愈52例,复发4例,其半年复查治愈率为92.86%;一年治愈50例,复发6例,其一年复查治愈率围89.29%;CINⅠ级半年及一年治愈例数均为44例,半年及一年治愈率均为95.65%;CINⅡ-Ⅲ级半年及一年治愈例数均为16例,半年及一年治愈率均为84.21%。结论液基薄层细胞学检测联合阴道镜检查对宫颈疾病的患者进行筛查、诊断具有重要价值,联合Leep术治疗宫颈病变临床效果好。是基层医院诊断治疗宫颈病变的理想模式。

Clinical Aplication of Thinprep Cytology Combined Colposcopy LEEP Operation in the Treatment of the Cervical Lesion

Objective To investigate the clinical application value of thinprep cytology combined colposcopy LEEP operation in the treatment of the cervical lesion. Methods 1026 patients in gynecology clinic of our hospital from September 2009 to February 2012 were selected as research object, and the patients with cervical lesions were detected with liquid based cytology test combined with colposcopy, the patients diagnosed as cervical lesions were treated with LEEP. Results 1026 patients were detected with liquid based cytology test combined with colposcopy, and 122patients were diagnosed as cervical lesions, the rate is 11.89%, 121 patients were treated with LEEP, the operative time 2-10min, mean operative time was （4.21~2.05） rain. Intraoperative blood loss was 2--45mL, the average intraoperative blood loss （9.56~6.57） mL. Incision healing time of 3-10 weeks, the average （6.52~3.20） weeks. Six months, 56 cases of chronic cervicitis, 52 cases were cured, 4 had recurrence. Its semi-annual review of the cure rate was 92.86%. Cured 50 cases of recurrence of six cases, its one-year review of the circumference 89.29% cure rate. CIN I level for six months and one year the number of cured cases were 44 cases, six months and one year cure rate was 95.65%. C1N II - III level for six months and one year the number of cured cases are 16 cases, six months and one year cure rate was 84.21%. Conclusion The liquid based cytology test combined with colposcopy has higher clinical value for the diagnosis of cervical lesions, and it is the it is the ideal model for the basic-level hospital.
