2型糖尿病患者并发非糖尿病性肾脏病变的病例特点及临床表现

目的研究2型糖尿病合并非糖尿病肾病(NDRD)的临床特点、常见病理类型,以及治疗方案。方法回顾性分析我院2006年9月至2012年9月期间住院行肾活检病理的2型糖尿病患者448例,其中非糖尿病肾病18例,糖尿病肾病(DN)22例,分析各组患者的人口学资料,临床及病理学资料,治疗经过,分析影响患者临床预后的的因素。结果 18例NDRD患者,年龄36~80岁,平均年龄52岁,男性12例,发生肾功能损伤2例,有视网膜病变者1例,有高血压者3例,22例DN患者,年龄35~82岁,平均年龄52岁,男性9例,发生肾功能损伤5例,有视网膜病变者7例,有高血压者6例。结论 2型糖尿病患者合并非糖尿病性肾病的比例较高,对于糖尿病病程较短患者,尽可能肾活检方可得到明确诊断。对于合并非糖尿病肾病的2型糖尿病患者,应尽早行免疫抑制治疗,病程进展与病理类型密切相关。

Clinical Research of Non-diabetic Renal Disease in Type 2 Diabetic Mellitus

Objective To investigate the clinical and pathological characters of Non-diabetic renal disease（NDRD） patients.Methods Retrospectively analyze the clinical information and pathological character of 448 type-2 diabetic mellitus,Diabetic nephropathy patients of 22;Non-diabetic renal disease patients of 18,factors of the patients in the department,during Sep 2006 to Sep 2012.Result 18 NDRD aged from 36 to 80,12 males.Renal injury happened in 2 patients,retinopathy in 1 patient,high blood pressure in 3 patients,22 DN,aged from 35 to 82,9 males.Renal injury happened in 5 patients,retinopathy in 7 patient,high blood pressure in 6 patients.Conclusion Renal biopsy was needed in NDRD patients with a short history of 2 diabetic mellitus,immunosuppressive therapy were necessary,Clincial manifestation of NDRD closely related with pathological character.
