消化系统恶性肿瘤血浆D-二聚体与癌胚抗原水平变化的临床意义

目的：通过对消化系统恶性肿瘤患者血浆中D-二聚体测定与血清癌胚抗原做对比,探讨其变化水平在临床判断病情和评估预后中的价值。方法：使用酶联免疫吸附双抗体夹心法原理定量测定90例消化道恶性肿瘤患者血浆D-二聚体（D-Dimer）水平,并选取50名健康人作为对照组,消化系统恶性肿瘤患者同时测定血清癌胚抗原水平。结果：与健康对照组相比,消化道恶性肿瘤组,血浆D-dimer水平明显升高（P〈0.01）。在胃癌无转移、淋巴结转移、远处脏器转移3组患者之间,血CEA浓度差异无统计学意义（P〉0.05）。而3组患者的血浆D-二聚体浓度差异有统计学意义（P〈0.05）。消化道恶性肿瘤转移与无转移组之间CEA及D-dimer比较,均有显著差异。结论：D-二聚体可作为消化系统恶性肿瘤患者病情评估的指标之一。

The Evaluation of Plasma D-dimer Levels on Carcinoma State of Digestive System

Objective. To study plasma D - dimer （ D - Di） value in evaluating carcinoma state in digestive system. Methods： The concentration of plasma D- Di and serum careinoembryonie antigen（CEA）was determined in 90 eases with carcinoma of digestive system and 50 normal control The correlation was analysed between the concentration of blood D - Di or CEA and the state of the patients with carcinoma of digestive system. Results. Compared with the control group, the concentration of plasma D - Di in the patients with carcinoma was higher significantly than that in control group（P 〈 0.01 ）. The concentration of serum CEA had no difference between patients with or without gastric cancer metastasis. But the concentration of plasma D- Di in patients with gastric cancer metastasis was higher than that in patients without gastric cancer metastasis（P 〈 0. 05）. The concentration of plasma D- Di and serum CEA in the patients with colonic carcinoma metastasis was higher than that in patients without colonic carcinoma metastasis（P〈 0.01）. Conclusion; D- Di is one of the indexes to evaluate the state of the patients with carcinoma of digestive system.
