纤维支气管镜技术在ICU中的应用研究

摘    要:目的探讨纤维支气管镜(简称纤支镜)技术在ICU中抢救治疗各种危重病患者的应用价值。方法采用中纤支镜治疗1CU 109例危重病患者,包括气管插管、机械通气过程中纤支镜检查、获取痰培养、吸痰、支气管冲洗、支气管肺泡灌洗、顽固性气胸封堵、代替胸腔镜治疗胸腔积液、止血治疗等情况。结果全部患者经纤支镜引导下经鼻气管插管,成功率99.17%,插管操作均在30s~3min内完成。442例次经纤支镜吸痰、支气管冲洗或支气管肺泡灌洗;306例次纤支镜取痰标本病原学培养,阳性率为92%;2例呼吸道出血致窒息患者经纤支镜止血治疗出血停止。1例顽固性气胸在纤支镜下封堵成功;1例胸腔积液在纤支镜直视下治愈。结论纤维支气镜技术在ICU中具有较大的临床应用价值。

Abstract:Objective To explore the value of Fibre Bronchoscope technology in salvage of various kinds emergency and severe cases in intensive care unit (ICU)ward. Methods To analysis retrospectively 109 severe cases' data who were treated by Fibre Bronchial Microscope, include of tracheal intubation, Fibre Bronchoscope check when mechanical ventilation, taking sputum for culture, aspiration sputum, irrigation of bronchus, bronchoalveolar Lavage, restore of refractoriness aeropleura, therapy of pleural effusion taking the place of thoracoscope, hematischesis and so on. Results All patients were intubated with nasal trachea cannula by aiming of Fibre Bronchoscope and achievement ratio was 99.17 %. The procedures were all finished in half to 3 minutes. The patiens were treated by Fibre Bronchoscope 442 times for aspiration sputum, irrigation of bronchus or bronchoalveolar lavage, 306 sputum samples were cultivated and the positive rate was 92%. 2 asphyxia cases because of hemorrhage from respiratory tract were healed by haemostasis with Fibre Bronchoscope; 1 refractoriness aeropleura and 1 pleural effusion case were cured. Conclusion Fibre Bronchoscope technology is of great value in ICU.
