2型糖尿病下肢血管病变患者的幽门螺旋杆菌感染分析

摘    要:目的探讨2型糖尿病患者幽门螺旋杆菌(Hp)感染与下肢血管病变之间的关系。方法比较131例2型糖尿病患者与24名健康对照者Hp感染率。131例2型糖尿病患者根据高分辨率彩色多普勒超声对下肢血管检查结果,分为无血管病变组71例、轻中度血管病变组32例、重度血管病变组28例,比较三组间的Hp感染率。并以有无下肢血管病变为因变量,Hp感染及已知动脉粥样硬化的危险因素为自变量,进行Logistic回归分析。结果2型糖尿病患者Hp感染率为49.6%,高于健康对照组(20.8%,〈0.05)。重度血管病变组Hp感染率89.3%(25/28),轻中度血管病变组Hp感染率56.3%(18/32),无血管病变组Hp感染率30.0%(22/71),三组之间两两比较差异有统计学意义(〈0.01)。Logistic回归分析提示Hp感染是2型糖尿病患者下肢血管病变的独立危险因素。结论2型糖尿病患者Hp感染率升高,Hp感染可能是2型糖尿病患者下肢血管病变的危险因素。

Abstract:Objective To study the relationship between vasculopathy in the lower extremities and helicobacter(Hp) infection in patients with type 2 diabetes. Methods The prevalence of lip infection in 131 type 2 diabetic patients and in 24 nondiabetic controls was compared, and then the 131 patients with type 2 diabete detected by high resolution color Doppler ultrasonography were divided into severe lower extremities vasculopathy group, mild lesion group, and no lesion group. The prevalenees of lip infection among three groups were compared. The risk factors of the lower extremities vasculopathy were studied in Logistic regression analysis models. Results The prevalence of lip infection in type 2 diabetic patients(49.6%) was higher than that in normal controls(20.8%). The prevalence of lip infection in severe lower extremities vasculopathy group(89.3%) was significantly higher than that in mild lesion group(53.1%)(P〈0.01), and the prevalence of Hp infection in mild lesion group(53.1%) was significantly higher than that in no lesion group(25.4%)(P〈0.01). Logistie analysis showed that Hp infection was an independent risk factor associated with lower extremities vaseulopathy. Conclusion The prevalence of Hp in diabetes patients is higher than that in the control. Hp infection may be a risk factor of the lower extremities vasculopathy in type 2 diabetic patients.
