未足月胎膜早破102例临床分析

目的:探讨未足月胎膜早破对母儿的影响及相关处理。方法:将102例未足月胎膜早破病例分为<28周、28～33+6周和34～36+6周3组,分析围产儿预后及母体并发症,比较未足月胎膜早破在不同孕周的差异。结果:胎儿窘迫发生率、新生儿窒息率、肺部并发症及感染发生率均以28～33+6孕周组高,分别为52.2%、65.2%和60.9%;<28孕周者围生儿死亡率(66.7%)明显高于其他两组。102例中母体感染56例,占54.9%。结论:未足月胎膜早破可致母儿严重并发症甚至危及生命,孕龄越小围产儿死亡率越高。母体感染难产是未足月胎膜早破母体常见并发症。

Clinical analysis of 102 cases with premature rupture of membrane

Objective:To explore the effect of premature rupture of membrane on mothers and infants and the corresponding treatment. Methods:102 cases with preterm premature rupture of membrane were divided into <28 gestational weeks group,28～33+6 gestational weeks group and 34～36+6 gestational weeks group.The prognosis of perinatal infants and the complications of mothers and infants were analyzed,the difference of preterm premature rupture of membrane was compared among different groups. Results:The incidence of fetal distress(52.2%),the incidence of neonatal asphyxia(65.2%),the incidence of pulmonary complications and infection(60.9%) in 28～33+6 gestational weeks group were the highest;the perinatal mortality in <28 gestational weeks group was 66.7%,which was significantly higher than those in the other two groups.Among 102 cases,56 cases were found with maternal infection,accounting for 54.9%. Conclusion:Preterm premature rupture of membrane can induce severe complications of mothers and infants,even threaten life,the earlier the gestational weeks is,the higher the perinatal mortality is.Maternal infection and dystocia are common maternal complications in the cases with preterm premature rupture of membrane.
