珠海地区妇幼保健院大肠埃希菌产ESBLs的基因型研究

目的 了解珠海地区妇幼保健院产超广谱β-内酰胺酶（ESBLs）大肠埃希菌基因特点.方法 收集珠海地区两家妇幼保健院临床分离的316株大肠埃希菌,用法国生物梅里埃公司VITEK-32全自动微生物分析仪检测其表型,再用双纸片法确定,所有产酶菌株用PCR的方法扩增其TEM,CTX M和SHV基因.结果 316株大肠埃希菌检出138株产超广谱β-内酰胺酶（ESBLs）,占43.7％（138/316）;138株产ESBLs大肠埃希菌PCR检测的结果为TEM型阳性的有72株,CTX M型阳性的有59株,SHV型阳性的有7株,检出率分别为52.2％（72/138）,42.8％（59/138）和5.1％（7/138）;同时携带TEM型和CTX M型的共23株,占16.6％（23/138）.结论 珠海地区妇幼保健院的大肠埃希菌产ESBLs的阳性率高,其基因型别主要以TEM型和CTX-M型为主,同一菌株可携带有两种型别的ESBLs.

Genotypes Research for E.coli Producing ESBLs in Zhuhai MCH

Objective To learn Zhuhai MCH extended spectrum β lactamases （ESBLs） phenotypes and genes characteristic of Escherichia coli （E.coli）.Methods Collected 316 strains of E.coli clinical isolated in zhuhai two maternity and child care.Used VITEK 32 automatic microorganism analyzer which produced by France bioMerieux company to test E.coli phenotype.Then confirmed by dual paper disk method,and PCR method was used for amplification of the TEM,CTX-M and SHV genes of all of the enzyme producing strains.Results 138 strains of ESBLs were found from the 316 strains of E coli,accounted for 43.7 ％ （138/316）.The results of the 138 strains of ESBLs were,72 strains of TEM positive,59 strains of CTX-M positive and 7 strains of SHV positive.Detection rates were 52.2％（72/138）,42.8％（59/138） and 5.1％（7/138） respectively.There were 23 strains with TEM positive and CTX M positive simultaneously,accounted for 16.6 ％ （23/138）.Conclusion There was a high positive rate of E.coli of ESBLs in Zhuhai MCH,and their genotypes mainly were TEM and CTX M.It would be possible that one strain carries two kinds of ESBLs.
