高龄股骨颈骨折患者腰硬联合麻醉的分析

摘    要:
目的:观察心肺功能较差的高龄股骨颈骨折患者腰硬联合麻醉(CSEA)的安全性及效果。方法:28例择期股骨颈骨折手术的高龄患者,夹杂心、脑、肺等多器官疾病,选择腰硬联合麻醉,对术中患者收缩压(SPB)、舒张压(DBP)、心率(HR)和脉搏氧(SPO2)等进行监测统计,术后观察视觉模拟评分(VAS)、并发症等。结果:患者麻醉注药后5min,SPB、DBP较注药前有显著性差异(P0.05)。术中面罩吸氧SPO2均大于96%。术后VAS评分均低于4分,无头痛、低氧血症、认知功能障碍等并发症。结论:对高龄股骨颈骨折合并心脑肺等多器官疾病的患者,采用腰硬联合麻醉是安全有效的。

Abstract:
Objective: To investigate the function of heart and lung poor fracture (CSEA) safety and effect. Methods: 28 patients undergoing elective operation of femoral neck fractures in elderly patients with heart, brain, lung, and other multi organ disease, combined spinal and epidural anesthesia for patients to choose, shrinkage during pressure (SPB), diastolic blood pressure (DBP), heart rate (HR) and pulse oxy- gen (SPO2) for monitoring statistics, postoperative observation VAS score, complications. Results: Anes- thesia drug injection in patients after 5min, SPB, DBP than that before injection had significant difference (P d0. 05). Intraoperative oxygen mask SPO2 is greater than 96%. VAS scores were lower than 4 points after operation, no headache, hypoxemia, cognitive dysfunction and other complications. Conclusion: Fracture complicated with cardiovascular and lung and other organs disease of elderly patients with femoral neck, u- sing combined spinal and epidural anesthesia is safe and effective.
