脐血中IGF-I和IGFBP-3与胎儿生长发育的关系

目的:探讨脐血中胰岛素样生长因子-I(IGF-I)和胰岛素样生长因子结合蛋白-3(IGFBP-3)的水平与胎儿生长发育的关系。方法:用免疫放射法测定146名新生儿脐血中IGF-I和IGFBP-3的含量。结果:(1)适于胎龄儿(AGA)脐血中的IGF-I和IGFBP-3的水平均低于大于胎龄儿(LGA)(P<0.001和P<0.05),高于小于胎龄儿(SGA)(P<0.01和P<0.001);(2)男女新生儿脐血中IGF-I和IGFBP-3的水平无明显差异(P>0.05);(3)出生时身长、体重、双顶径、足长和胎盘重量这5个生长参数均与IGF-I和IGFBP-3呈显著正相关(均P<0.001);(4)AGA、LGA脐血中IGF-I和IGF-BP-3呈显著正相关(P<0.01和P<0.001),但这种相关性在SGA中不存在(P>0.05)。结论:IGF-I和IGFBP-3与胎儿宫内生长发育密切相关,为宫内发育迟缓儿生后的早期发现和早期治疗提供临床指导。

The relationship between IGF-I and IGFBP-3 in umbilical cord serum and human fetal growth

Objective:To study the relationship between insulin-like growth factor-I(IGF-I) and insulin-like growth factor binding factor-3(IGFBP-3) in umbilical cord serum and human fetal growth.Methods:The levels of IGF-I and IGFBP-3 in umbilical cord serum of 146 neonates were assayed by IRA.Results:(1) The levels of IGF-I and IGFBP-3 in appropriate for gestational age(AGA) were lower than those of large for gestational age(LGA)(P <0.001and P<0.05) and higher than those of small for gestational age(SGA)(P<0.01 and P<0.001).(2) There was no difference in IGF-I and IGFBP-3 between male and female fetus.(3) There were positive correlations between length,birth weight,BPD,foot length and placental weight and IGF-I and IGFBP-3(all P<0.001).(4) The levels of IGF-I and IGFBP-3 in umbilical cord serum of AGA and LGA were significantly positive correlations(P<0.01 and P<0.001),but the correlation was not seen in SGA.Conclusion:The IGF-I and IGFBP-3 are closed to human fetal growth.They could instruct to diagnose and treat intrauterine growth retardation(IUGR) earlier.
