腹腔镜胆总管探查术69例临床分析

目的：探讨分析腹腔镜胆总管探查术（LCBDE）的临床应用价值。方法将我院收治的69例确诊为胆总管结石患者按照随机数字表法分为观察组（采用LCBDE治疗）和对照组（采用传统开腹胆总管探查术（OCBDE）治疗），对比观察两组患者手术时间、胃肠道功能恢复时间、术后镇痛次数、住院时间、住院费用等手术情况、二次手术以及术后并发症发生情况。结果两组患者手术时间以及术后并发症发生率并无明显差异（P〉0.05）。观察组患者胃肠道功能恢复时间、术后镇痛次数、住院时间、住院费用分别为（29.9±5.7）、（1.3±0.5）、（7.5±1.9）；对照组患者胃肠道功能恢复时间、术后镇痛次数、住院时间、住院费用分别为（34.9±5.8）、（1.7±0.7）、（10.6±3.0），两组比较，观察组均显著优于对照组（P〈0.05）；观察组二次手术率（0%）明显低于对照组（8.8%），差异对比具有统计学意义（P〈0.05）。结论腹腔镜胆总管探查术的临床疗效确切，对患者的创伤较小，手术成功率较高，有利于缩短住院时间，减轻患者家庭经济负担，值得在临床上广泛推广和应用。

Laparoscopic common bile duct exploration, 69 cases of clinical analysis

Objective To investigate the clinical value analysis of laparoscopic common bile duct exploration （LCBDE）＇s. Methods 69 cases in our hospital patients diagnosed with bile duct stones randomly divided into observation group （using LCBDE therapy） and control group （using the traditional open common bile duct exploration （OCBDE） treatment）, compared to observe two patients operative time, time to recovery of gastrointestinal function, the number of postoperative pain, length of stay, hospital costs, such as surgery, the second surgery and postoperative complications were observed. Results The two groups were operative time and postoperative complication rate was not significantly different （P〉0.05）, but the observation group were gastrointestinal function recovery time, the number of postoperative pain, length of stay, hospital costs, and the second surgery aspect ratio, etc. were significantly better than the control group （P〈0.05）. Conclusion The clinical efficacy of laparoscopic common bile duct exploration precise, less trauma to the patient, the higher the success rate of surgery, help shorten hospital stays, reduce the financial burden of families of patients, should be popularized and applied in clinical practice.
