COPD所致重度呼吸衰竭应用BiPAP无创通气治疗的临床观察

目的探讨分析COPD合并呼吸衰竭应用BiPAP无创通气治疗的效果。方法取我院2011—2012年收治的80例COPD合并呼吸衰竭患者进行分析,将患者随机分成观察组和对照组,每组40例,对照组给予常规抗炎、持续低流量吸氧（2L/min）、解痉平喘、肾上腺皮质激素、纠正酸碱平衡等治疗;观察组在常规治疗的基础上给予BiPAP无创通气治疗,对比两组患者治疗效果。结果观察组患者治疗总有效率达95%,对照组患者治疗总有效率达67.5%,组间比较差异具有统计学意义（P〈0.05）;观察组患者治疗后的PaO2值为（72.9±7.3）mmHg,明显高于对照组患者的（63.8±5.1）mmHg,组间比较差异具有统计学意义（P〈0.05）。结论 COPD合并呼吸衰竭患者给予BiPAP无创通气治疗具有显著疗效,并明显优于常规治疗,值得在临床上推广应用。

Clinical observation of COPD in severe respiratory failure caused by application of BiPAP non-invasive ventilation treatment

Objective To explore the application of BiPAP analysis of COPD complicated with respiratory failure noninvasive ventilation therapy.Methods Analysis of our hospital in 2011-2012 80 cases of COPD complicated with respiratory failure patients,the patients were randomly divided into observation group and control group,40 cases in each group,the control group was given conventional anti-inflammatory,persistent low flow oxygen（2L/min）,antispasmodic,adrenal cortical hormone,correction of acidbase balance treatment; the observation group in on the basis of routine treatment given BiPAP non-invasive ventilation treatment,compared two groups of patients the effect.Results The patients in the observation group the total effective rate of the therapy of 95% patients in the control group,the total effective rate of the therapy was 67.5%,with statistical significance between the two groups（P〈0.05）; the patients in observation group after treatment PaO2 value was（72.9±7.3） mmHg,significantly higher than that in the control group were（63.8±5.1） mmHg,statistically significant difference between the two groups（P〈0.05）.Conclusion The patients with COPD complicated with respiratory failure received BiPAP non-invasive ventilation treatment has obvious curative effect,and is superior to conventional treatment,worthy of clinical application.
