老年患者铜绿假单胞菌感染及耐药监测

摘    要:目的 分析铜绿假单胞菌引起老年患者感染的特点及对 2 2种抗菌药物的耐药谱 ,为临床治疗用药提供合理方案。方法 用VITEK32型全自动细菌分析系统对我院老年住院患者临床分离细菌进行鉴定 ,共分离出 1 70株铜绿假单胞菌 ,并采用Kirby Bauer法选用 2 2种抗菌药物进行体外药物敏感试验。结果  1 70株铜绿假单胞菌主要来源于痰标本 ,占 76 .5 % ;其在临床病区分布中居前 2位者依次为呼吸内科、老年病科。 1 70株铜绿假单胞菌对阿米卡星、左氧氟沙星、环丙沙星、哌拉西林 /他唑巴坦、亚胺培南和妥布霉素的耐药率较低 ,均小于 40 % ;其对氯霉素、复方新诺明、氨苄西林、头孢噻吩、萘啶酸、阿莫西林 /克拉维酸和头孢西丁则表现出高度耐药。结论 铜绿假单胞菌是引起医院感染的最常见病原菌之一 ,尤易侵袭老年患者 ,对抗菌药物呈多重耐药 ,临床治疗应结合老年患者的特点合理选用抗菌药物 ,以减少耐药株的出现与扩散

Abstract:Objective To investigate the distribution of multi drug resistance of pseudomonas aeruginosa (PA) among old patients and to analyze the characteristic of the PA infection. Methods VITEK32 was used for the identification of 170 PA clinical isolates and the resistance of 22 kinds antibiotics was determined by Kirty Bauer method. Results Most of 170 strains were isolated from sputum (77.6%). 170 strains were mainly isolated from department of respiratory diseases and geriatrics department. The resistance rates of amicaxin, levofloxacin, ciprofloxacin, piperacillin tazobactam, imipenem and tobramycin were respectively below 40%. PA was highly resistant to chloramphenicol, SMZ, ampicillin, cephalothin, nalidixic acid, amoxillin clavulanic acid and cefoxitin.Conclusions PA is one of the most common pathogens causing nosocomial infection especially for senile patients. Their susceptibility to antibiotics showed multidrug resistance. In order to reduce or prevent the occurrence of resistant isolate, we should rationally choose and use antibiotics combining with trait of senile patients.
