| Abstract: | Health management in grass roots of type 2 diabetes(T2DM)patients,as an important measure of equalization of basic public health services,has achieved initial results,but also faces new opportunities and challenges.Using ROCCIPI method to analyze the existing problems of grass-roots health management for T2DM patients from the aspects of rules,process,opportunities,capacity,communication,interests,process and awareness.Based on it,we propose improvement strategies such as establishing support soft and hard environment,constructing management model of“medical and prevention integration”,strengthening diabetes standardized management training and popularizing self-management health education for diabetic patients,in order to provide reference for optimizing the health management of T2DM patients at the grass roots. |
