人参多糖对创伤脓毒症患者免疫功能和细胞因子的影响

摘    要:
目的研究人参多糖对创伤脓毒症患者炎症因子及免疫功能的影响,探讨人参多糖在外科感染治疗中的作用。方法 48例创伤脓毒症患者随机分为治疗组24例和对照组24例,对照组采用经典SSC治疗,治疗组在上述治疗基础上加用人参多糖,疗程为7 d。分别观察治疗前和治疗后2组患者相关临床资料和生存分析,比较APACHEⅡ评分,血浆内毒素,肿瘤坏死因子(TNF-α),白细胞介素-6(IL-6),白细胞介素-10(IL-10),CD14+单核细胞HLA-DR,NK细胞和T细胞亚群CD4+、CD8+、CD4+/CD8+百分率的变化。结果治疗组患者机械通气和使用升压药的时间较对照组均明显缩短(P均<0.05);2组APACHEⅡ评分治疗前比较无显著性差异,治疗后第7天时治疗组明显低于对照组(P<0.05)。2组之间28 d病死率比较无显著性差异(P>0.05)。与对照组比较,治疗组内毒素、TNF-α、IL-6水平明显下降,IL-10水平明显升高,CD14+单核细胞HLA-DR,NK细胞和T细胞亚群CD4+、CD8+、CD4+/CD8+均明显上升(P均<0.05)。结论人参多糖可以减轻创伤脓毒症患者的炎症反应,改善其免疫功能,缩短机械通气和使用升压药的时间。
