常规超声心动图、心电图、动态心电图及核素心肌灌注断层显像对冠心病诊断的对比研究

摘    要:目的对比分析常规超声心动图(UCG)、心电图(ECG)、动态心电图(Holter)及核素心肌灌注断层显像(SPECT)对冠心病的诊断价值。方法回顾性分析120例冠心病患者的UCG、ECG、Holter及SPECT检查结果,并与冠状动脉造影(CAG)检查结果作对照。结果 120例患者中,CAG检查98例患者为冠心病,UCG显示82例患者室壁节段运动异常,ECG显示68例患者ST-T段改变,Holter显示74患者例呈阳性,SPECT显示90例患者显像异常。UCG、ECG、Holter及SPECT诊断冠状病的敏感性和特异性分别为79.59%、60.20%、65.31%、83.67%和81.82%、59.09%、54.54%、63.64%。结论 UCG的敏感性略低于SPECT,但特异性明显高于其他方法,对冠心病的诊断有较高的准确性,值得临床的广泛应用。

Abstract:To comparative analyze the value of eehocardiography and electrocardiography (ECG) , Holter, single photon emission computed tomography (SPECT) in diagnosis of coronary heart disease (CHD). Methods The findings of echocardiography, ECG, Holter and SPECT were retrospectively analyzed in 120 cases diagnosed as CHD , and the results were compared with those of CAG. Results Among the 120 cases ,98 cases of CHD were confirmed by CAG, eehocardiography showed regional wall motion abnormality in 82 cases, ECG showed STT changes in 68 cases, Holter showed positive results in 74 cases, and SPECT showed abnormal imaging in 90 cases. The sensitivity and specificity of echoeardiography and ECG, Hoher, SPECT in diagnosis of CHD were 79.59% , 60.20% , 65.31% ,83.67% and 81.82% , 59.09% , 54.54% ,63.64% respectively. Conclusion The sensitivity of eehocardiography is slightly lower than SPECT, but the specificity is significantly higher than other methods. It has higher accuracy in diagnosis of CHD, which is worth applying widely in clinic.
