金瓷桥3种桥体龈端形态的临床应用

目的：探讨金瓷桥3种桥体龈端形态的临床应用效果。方法：选择门诊待行单个后牙双端烤瓷固定桥修复的60例患者，将烤瓷桥体龈端形态随机分为改良鞍式桥体、改良盖嵴式桥体、舟底式桥体3组，按所设计桥体形态的要求完成修复体，3个月后取下修复体。观察3组患者戴用烤瓷桥体前后桥体龈端形态异物感、发音、美观效果以及桥体接触区发生粘膜红肿、炎症的情况。结果：60例患者仅5例出现异物感，戴用改良鞍式桥体的患者未出现明显异物感，戴用改良盖嵴式桥体的患者出现异物感4例（6．7％）。以舟底式桥体粘膜红肿发生率最低．炎症程度最轻，改良鞍式桥体粘膜红肿发生率最高，炎症程度最重，3组患者比较差异有统计学意义（x^2=5．42，P〈0．05）。结论：舟底式桥体优于改良鞍式桥体和改良盖嵴式桥体，是值得临床推广应用的一种更优化的桥体设计。

Analysis of clinical applying on the form of three types of pontics tissue surface of the ceramometal bridge

Objective: To investigate the clinical effects of the application on three ceramometal pontics types. Methods:Sixty patients who were ready to accept fixed partial denture about three unite bridge were selected and divided into three groups at random: modified base-type pontic, modified ridge lap pontic and the ovate pontic. 3 months after insertion fixed partial dentures were observed in order to compare to the effects of three types of pontic. Results: After three months wearing ceramometal bridge, there were statistical differences (P<0. 05) among the three groups about comfort, gingivitis; while no difference was found about esthetic and pronunciation (P >0. 05). Conclusions: The ovate pontic has a less influence than the other two types and is a more appropriate pontics design type for dentist.
