摘    要:
目的 探讨脑卒中患者合并医院获得性肺炎(HAP)病原菌的耐药性和感染危险因素,为临床预防和治疗疾病提供科学依据.方法 细菌培养按照《全国临床检验操作规程》进行;药敏试验采用K-B法.结果 HAP 的主要病原菌为肺炎克雷伯菌、铜绿假单胞菌、鲍氏不动杆菌、金黄色葡萄球菌,分别占24.5%、17.8%、13.3%、14.3%;MRSA检出18株,检出率为43.9%,鲍氏不动杆菌及铜绿假单胞菌泛耐药菌株分别检出9、13株,检出率分别为23.7%、25.5%;病原菌均显示多药耐药性.结论 临床应针对危险因素,采取综合性干预措施,预防与控制脑卒中患者医院感染.

Abstract:
OBJECTIVE To investigate the risk factors for infections and antimicrobial resistance of pathogens causing hospital acquired pneumonia(HAP) in stroke patients and to provide the scientific reference for clinical prevention and treatment of diseases.METHODS Referring to National Guide to Clinical Laboratory Operation Procedures,the strains were cultured.The antibiotic susceptibility testing was performed by K-B methods.RESULTS The main pathogens causing HAP were Klebsiella pneunmoniae,Pseudomonas aeruginosa,Acinetobacter baumannii and Staphylococcus aureus,accounting for 24.5%,17.8%,13.3%,and 14.3%,respectively.There were 18 strains(43.9%) of MRSA detected;9 strains of A.baumannii and 13 strains of P.aeruginosa were detected with the detection rates of 23.7% and 25.5% respectively.The isolates were all multi-drug resistant.CONCLUSION Clinicians should take comprehensive intervention measures for the risk factors to prevent and control HAP in the stroke patients.
