乙型肝炎五项标志物定量检测的临床分析

目的 探讨酶联免疫吸附法(ELISA)和微粒子化学发光法检测乙型肝炎表面抗体(HBsAb)的结果并进行对比分析.方法 每份标本均用ELISA和MCLIA两种方法,同时进行血清学标志物的定量和定性检测,均严格按照各自试剂盒说明书和《全国临床检验操作规程》的要求进行操作,定性结果参照试剂说明书中的判定方法.结果 MCLIA法和ELISA法检测乙型肝炎病毒标志物结果比较,HBsAg、HBsAb、HBeAg和HBcAb结果符合率较高均＞90.0％,差异无统计学意义,HBeAb结果符合率均为47.7％,符合率较低,差异有统计学意义(P＜0.05)；MCLIA法和ELISA法检测HBsAg含量符合率＞90.0％.结论 用MCLIA检测乙型肝炎血清标志物具有灵敏度高、重复性和准确性好、特异性强、检测快速、无放射性危害等优点,对于少见模式、可疑的低值血清检测效果更好.

Clinical analysis of quantitative detection for Hepatitis B

OBJECTIVE To compare enzyme-linked immunosorbent assay(ELISA) and microparticle chemiluminescence in the detection of hepatitis B surface antibody(HBsAb).METHODS Serological markers were detected quantitatively and qualitatively in each specimen both by the ELISA and MCLIA in accordance with their respective kit instructions and the "national clinical laboratory operating procedures".The qualitative results were recorded according to the kit instructions of determination method.MCLIA and ELISA detection of hepatitis B virus markers were compared.RESULTS Coincidence rates of HBsAg,HBsAb,HBeAg and HBcAb were higher than 90.0%,the difference was not statistically significant(P>0.05).Coincidence rates of HBeAb results was 47.7%,the difference was statistically significant(P<0.05).CONCLUSION Coincidence rates were 90.0% in detection of HBsAg levels of both MCLIA and ELISA.MCLIA detection of hepatitis B serum markers was high sensitivity,repeatability and accuracy,specificity,rapidity and non-radiative.It is better at detection of low values in serum.
