耐碳青霉烯类铜绿假单胞菌的感染特征和危险因素分析

目的 研究耐碳青霉烯类铜绿假单胞菌感染特征、预后及危险因素。方法 回顾性病例对照1:4配比分析2012年1月－2017年2月，耐碳青霉烯类与非耐碳青霉烯类铜绿假单胞菌感染患者预测耐碳青霉烯类铜绿假单胞菌感染的危险因素。结果 耐碳青霉烯类铜绿假单胞菌常见感染部位是呼吸道感染，占59.80%，主要分布在重症监护室(ICU)(48.04%)和神经外科(26.47%)；机械通气、手术治疗、感染前使用抗菌药物、碳青霉烯类抗菌药物、多药联类和入住ICU是耐碳青霉烯类铜绿假单胞菌组感染的独立危险因素，治疗有效率仅为59.80%。结论 耐碳青霉烯类铜绿假单胞菌感染形势严峻，预后差；注意呼吸道重症感染的防控、减少不必要侵入诊疗、合理使用抗菌药物等是减少其感染，提高疗效措施。

Analysis of infection characteristics and risk factors of carbapenem resistant Pseudomonas aeruginosa

Objective To study the characteristics, prognosis, and risk factors of carbapenem resistant Pseudomonas aeruginosa. Methods A retrospective case-control study: 1:4 ratio analysis in January 2012-February 2017 of carbapenem-resistant and non-resistant infections in patients with risk factors for the prediction of infections of carbapenem-resistant Pseudomonas aeruginosa. Results The carbapenem-resistant Pseudomonas aeruginosa infection is a common respiratory infection, accounted for 59.80%, and mainly distributed in the ICU (48.04%) and the Department of Neurosurgery (26.47%); mechanical ventilation, surgical treatment, infections before the use of antibiotics, usage of carbapenem antibiotics, usage of multi drugs, and being in ICU are the independent risk factors of carbapenem-resistant Pseudomonas aeruginosa infections in the treatment group. The effective rate was only 59.80%. Conclusion Carbapenem-resistant Pseudomonas aeruginosa infection situation is grim and poor prognosis.
