摘    要:目的:探讨牙列缺损、缺失及重度磨耗患者咬合重建治疗对改善其咀嚼功能和面容美观的影响。方法:回顾临床牙列重度磨耗、牙列缺损或缺失40例病例,采用固定修复或固定活动联合修复方法实施咬合重建。咬合重建前确定治疗计划,余留牙进行完善的牙髓、牙周治疗;确定颌位关系后戴用过渡性垫3个月后固定义齿或固定活动联合修复。通过系列治疗恢复正常的垂直距离、曲线、平面,重建良好稳定的咬合关系。结果:咬合重建治疗后患者外形容貌、发音、咀嚼效能主观评价良好,咬合关系和颞下颌关节紊乱症状有显著改善。结论:采用固定修复或固定活动联合修复方法治疗牙列重度磨耗,牙列缺损或缺失,进行咬合重建的效果令人满意。详尽的治疗计划、正确的颌位关系确定、过渡性义齿戴用是固定修复咬合重建成功的关键。

Abstract:Objective To investigate the effects of occlusal reconstruction on improving masticatory efficiency and facial beauty of patients suffered from serious attrition with dentition defect of dentition missing.Methods 40 cases of patients of serious attrition with dentition defect or missing were evaluated.Fixed partial denture or fixed removable combined denture were used to reconstruct occlusion.Complete treatment plan was made before the beginning of occlusal reconstruction.The residual teeth were treated.Vertical dimension and occlusal plane were reconstructed and occlusion relationship was improved.Results The masticatory efficiency and facial beauty were improved significantly and TMJ disorder was alleviated.Conclusion Fixed partial denture or fixed removable combined denture can be used to reconstruct pathological occlusion and the curative effect is satisfactory.Proper treatment plan,accurate occlusion relationship and the use of transient denture were important to the success of occlusal reconstruction.
