血浆N-端脑利钠钛前体和心钠素水平在慢性肺源性心脏病中的临床意义

目的探讨慢性肺源性心脏病患者血浆N-端脑利钠钛前体（ET-proBNP）和心钠素（ANP）的水平及临床意义。方法对46例肺源性心脏病患者及42例正常受试者采用快速化学发光法测定血浆ET—proBNP、ANP，并行同步动脉血气分析、肺功能检查，同时应用超声波检测收缩期三尖瓣返流压力价差（TRPG）估测肺动脉压（PAP）。结果慢性肺源性心脏病患者NT—proBNP、ANP均显著高于对照组（P〈0．01），且血浆NT—proBNP水平与ANP、PAP呈正相关，与PaO2、FEV1、MMEF呈负相关（均P〈0．05）。结论血浆NT—proBNP和ANP共同参与了肺源性心脏病的病理生理过程，测定NT-proBNP和ANP水平的变化有助于判断肺源性心脏病病情、严重程度和估计预后。

Change and significance of plasma N-terminal prohormone brain natriurctic peptide and Arterial natriuretic peptide with chornic pulmonary heart disease

Objective To study the plasma level of NP-proBNP and ANP and its signfiance in patients with chronic pulmonary heart disease. Methods The plasma level of NT-ProBNP and ANP were measude in 46 patients with chronic pulmonary heart disease and 42 heahhy persons by using chemiluminescence. Simuhaneouslg, artery blood gas analysis and lungfunction tests was done. The tricuspid valve regurgitation pressure gradient biggest （TRPG） in calculating pulmonary artery pressure was used in control group. Results The levels of NT-proBNP and ANP for chronic pulmonary heart diseare group were both highet than that in the normal control group （ P 〈0. 01 ）. The plasm level of NT-proBNP with the ANP,PAP,positively correlated and PaO2, FEV1 ,MMEF was negatively correlated （ all P 〈 0.05 ）. Conclusion The results suggest that NT-proBNP and ANP are involved in the pathophysiological process of PHD. Detetming the level of NT-proBNP and ANP could be helpful to judgement the severity degree and estimate prognosis in PHD.
