剖宫产术后应用布托啡诺复合甲磺酸罗哌卡因硬膜外自控镇痛

目的观察布托啡诺复合甲磺酸罗哌卡因在剖宫产患者术后硬膜外自控镇痛(PCEA)的效果和副反应。方法100例(美国麻醉医师协会病情评估分级Ⅰ~Ⅱ级)剖宫产患者随机分为2组,每组50例。观察组术后给予布托啡诺15 mg.L-1复合1.79 g.L-1甲磺酸罗哌卡行PCEA,对照组给予吗啡20 mg.L-1复合1.79 g.L-1甲磺酸罗哌卡因行PCEA。以视觉模拟评分法(VAS)评估术后疼痛并记录患者满意度和副反应。结果与对照组比较,观察组术后2 h和8 h静息和咳嗽时的VAS评分、镇痛泵按压次数、恶心呕吐发生率明显降低(P<0.05),患者镇痛满意率明显升高(P<0.05)。结论15 mg.L-1布托啡诺复合1.79 g.L-1罗哌卡因可安全有效地用于剖宫产术后的PCEA。

Postoperative epidural analgesia after caesarean section with butorphanol plus ropivacaine

Objective To observe the analgesia and side effects of butorphanol or morphine combined with ropivacaine in post-caesarean section patients with patient controlled epidural analgesia （PCEA）. Methods A hundred （ ASA I - Ⅱ） cases underwent caesarean section were randomly divided into two groups, fifty cases in each group. Butorphanol 15 mg·L^-1 （experiment group） or morphine 20 mg·L^-1（ control group） with 1.79 g·L^-1 ropivacaine were given epidurally after caesarean section respectively. Postoperative pain,patients＇satisfaction level and side effects were assessed by using visual analogue scale （VAS）. Results Compared with control group, the VAS scores of 2 h and 8 h after operations, the number of effective consumption, incidences of nausea and vomiting in experiment group were significantly lower（P 〈 0.05 ） ;But the satisfactory rate of analgesia was higher （ P 〈 0.05 ）. Conclusion 15 mg·L^- 1 butorphanol with 1.79 g·L^- 1 ropivacaine could be safe in PCEA after caesarean section.
