拉莫三嗪对双相抑郁患者认知功能的影响

目的 评估拉莫三嗪对双相抑郁（bipolar depressive disorder,BDD）患者认知功能的影响.方法 应用拉莫三嗪对35例BDD患者治疗6周.采用汉密尔顿抑郁量表（Hamilton rating scale for depression,HAMD,17项）评定临床症状;应用威斯康辛卡片分类测验（Wisconsin Card Sorting Test,WCST）和数字广度测验评估患者认知功能,并与30例正常对照进行比较.结果 治疗后HAMD评分明显改善（P＜0.01）,有效率为62.86％.研究组治疗前WCST、数字广度得分均较正常组差（P＜0.01）;治疗后持续错误数、完成分类数、数字广度-顺背仍较正常组差（P＜0.05）.患者认知功能的改善与临床症状的改善（疗效）无相关性.结论 拉莫三嗪能改善BDD患者的临床症状,但对BDD患者的认知功能无明显影响.

The Effects of Lamotrigine on Cognitive Function in Bipolar Depressive Disorder

Objective To investigate the effects of lamotrigine on cognitive function in bipolar de- pressive disorder.Methods 35 patients of bipolar depressive disorder were given lamotrigine for six weeks.The clinical efficacy was evaluated with the Hamilton Depression Scale-17 items. The cognitive function of the patients was assessed by the Wisconsin Card Sorting Test （WCST） and the digit span test and compared with those of the 30 patients in the normal group. Results The scores of HAMD at the endpoint were lower than those at the baseline （P〈 0.01 ）, and the response rate was 62.86 %. Before treatment, the scores of cognitive tests in the patients＇ group were significantly lower than those in the normal group （P〈0.0l）.After treat- ment,the numbers of preservative errors ,categories and the score of digit span-forward were still different from those in the normal group （P〈0.05）.The improvement of the cognitive func- tion was negatively correlated with decrease rate of HAMD.Conclusion Lamotrigine can improve the symptoms of bipolar depressive disorder patients without significant cognitive impairments.
