老年脑卒中患者抑郁量表筛查及相关心理治疗的临床分析

目的探讨老年脑卒中患者抑郁筛查情况及相关心理治疗的影响。方法将首次脑卒中发病1个月后的患者80例作为治疗组,另外选择同期住院的老年脑卒中抑郁患者74例作为对照组。两组患者均进行常规治疗,对照组每日口服百忧解20 mg,治疗组在此基础上进行针对性的心理治疗,分析脑卒中后患者抑郁的因素,于治疗前、治疗8周和6个月后根据汉密顿抑郁量表(HAMD)和抑郁自评量表(SDS)进行抑郁情况判断,进行HAMD、SDS评分和脑卒中神经功能缺损情况(CSS)比较,并统计治疗1年后的临床疗效。结果治疗组患者在治疗后8周和6个月的HAMD、SDS和CSS评分均低于治疗前,差异有统计学意义(P0.05)。治疗1年后,临床有效率达到91.25%,与对照组比较差异有统计学意义(P0.05)。结论对脑卒中患者进行抑郁量表筛查和相关心理治疗指导,对缓解抑郁症状、减轻家庭和社会负担有重要意义。

Clinical analysis of old patients with post-stroke depression screening and psychological treatment

Objective To investigate the screening elderly stroke patients with depression and related psychological therapy. Methods 80 patients with first time stroke one month ago as treatment group, and 74 inpatients with post-stroke depression as control group. Two groups all carried out conventional treatment with oral Prozac 20 mg daily, and treatment group was supplemented with psychological therapy. Depression factors of patients after stroke, before treatment, 8 weeks and 6 months after treatment were analyzed with the Hamilton depression scale （HAMD） and self rating depression scale （SDS）. HAMD, SDS score and stroke neurological deficits （CSS） were compared after one year. Results 8 weeks and 6 months after treatment, HAMD score, SDS score and CSS score were lower than the scores before treatment. After 1 year of treatment, the HAMD, SDS score and stroke neurological deficits （CSS） were significantly lower than before with a clinical effective rate of 91.25% （P〈0.05）. Conclusion It is important to carry out stroke depression scale screening for the patients after the first stroke and carried out the psychological treatment guidelines. Early treatment can reduce their depression and the family and society burdens.
