动态增强MRI对前列腺癌的分期作用

目的 探讨动态增强MRI对前列腺癌分期的作用。资料与方法 对37例前列腺癌患者行动态增强MRI检查,增强后图像与常规SET1W,FSET2W图像比较,主要评价包膜外侵犯包括精囊受累情况。结果 37例中包膜外侵犯28例,其中18例动态增强较T1WI,T2WI提供了更多的病灶情况。动态增强确定而常规序列图像不能肯定3例;精囊侵犯23例中动态增强观察更为明确13例,常规序列难以肯定。动态增强确定2例。根据增强图像有3例更改了病灶分期。结论 动态增强有助于综合判断部分前列腺癌的分期。

Evaluation of Dynamic Enhanced MRI in the Staging of Prostate Carcinoma

Objective To evaluate dynamic enhanced MRI in the staging of prostate carcinoma.Materials and Methods Dynamic enhanced MRI using bolus injection technique was performed in 37 patients with prostate carcinoma. Comparison, focusing on extracapsular spread and spermatocystic involvement, between dynamic enhanced and conventional SE T 1WI, FSE T 2WI was made.Results Of 37 cases, extracapsular involvement was seen in 28, in 18 of which dynamic enhanced MRI provided more information about the lesion than T 1WI or T 2WI did. In three cases, the extracapsular involvement could be confirmed only on dynamic enhanced MRI. Involvement of seminal vesicle was found in 23 patients, of which the involvement was more definite or could be only determined on dynamic enhanced MRI in 13 and 2, respectively. Based on enhanced MRI signs, the initial staging was changed in 3 cases.Conclusion Fast dynamic enhanced MRI can clearly display the extracapsular spread and spermatocystic involvement, thus, it is helpful for the staging of prostate carcinoma.
