妇科手术患者并发血栓的危险因素分析及护理干预措施

目的探讨妇科手术患者并发血栓的危险因素及护理干预措施。方法选取2018年1月至2019年12月在我院妇科住院的手术患者1623例,根据患者的临床资料分析发生血栓并发症的危险因素,并制定针对性预防护理措施。结果1623例妇科手术患者中发生血栓并发症184例,发生血栓和未发生血栓的患者年龄、BMI、文化程度、合并基础疾病、凝血功能异常、术后卧床时间、饮水情况比较,差异有统计学意义(P<0.05);Logistic回归分析结果显示,患者年龄、BMI、合并基础疾病、凝血功能异常、患者术后卧床时间、饮水情况均为手术患者发生血栓的独立危险因素。结论妇科手术患者并发血栓的相关危险因素较多,护理人员应加强对患者的高风险评估、筛查及预防性护理的健康宣教及督导落实,重视家庭成员的共同干预,尽量降低血栓发生的风险及相关并发症,提高患者的安全及生活质量。

Analysis of risk factors of complicated thrombosis and nursing intervention measures in gynecological surgery

Objective To explore the risk factors and prepare the nursing intervention measures in gynecological surgery patients complicated with thrombosis.Methods From January 2018 to December 2019,1,623 patients with gynecological surgery in our hospital were selected.According to the clinical data of patients,the risk factors of thrombosis complications were analyzed,and targeted preventive nursing measures were formulated.Results There were 184 patients with thrombotic complications in the 1,623 gynecological surgery patients.There were statistically significant differences in age,BMI,education level,combined basic diseases,abnormal coagulation function,postoperative bedridden time and drinking water between patients with and without thrombosis(P<0.05).Logistic regression analysis showed that age,BMI,combined basic diseases,abnormal coagulation function and postoperative bed time and drinking water were independent risk factors for thrombosis in the surgical patients.Conclusion There are many risk factors related to thrombosis in gynecological surgery patients.Nursing staff should strengthen the health education and supervision of patients with high risk assessment,screening and preventive nursing.At the same time,nursing staff should attach importance to the joint intervention of family members,so as to reduce the risk of thrombosis and related complications,and improve the safety and quality of life of patients.
