慢性稳定性心绞痛合并糖尿病患者循环microRNA-92a表达研究

摘    要:
目的了解循环miR-92a在慢性稳定性心绞痛(SAP)合并2型糖尿病患者中的表达,探讨循环miR-92a在冠心病临床应用的可能性。方法分析比较慢性稳定性心绞痛合并2型糖尿病患者、SAP无糖尿病合并症患者循环miR-92a表达。结果方差成份分析,糖尿病对SAP循环miR-92a表达的贡献(偏差平方和16.676)是年龄对SAP循环miR-92a表达贡献(偏差平方和2.474)的6倍多。以年龄为协变量,协方差分析SAP合并糖尿病病例入院次日循环miR-92a表达水平显著高于SAP无糖尿病合并症病例(F=21.86,P=0.001)。SAP合并糖尿病病例多支血管病变发生率73.3%(22/30)显著高于SAP无糖尿病合并症病例多支血管病变发生率51.2%(44/86)(χ2=4.458,P=0.035)。结论 SAP合并糖尿病病例循环miR-92a表达增高。SAP合并糖尿病病例较无糖尿病病例冠脉病变严重。提示针对循环miR-92a表达增高的合并糖尿病的SAP进行干预治疗的临床实际应用价值值得进一步深入研究。

Abstract:
【Objective】 To investigate circulatory level of microRNA 92a(miR-92a) in patients with stable angina pectoris(SAP) complicated with type 2 diabetes mellitus(T2DM).【Methods】 To determine the regulation of miR-92a,we detected it in peripheral blood mononuclear cells of patients of SAP with or without T2DM.Based on the circulating miR-92a in patients with SAP,the influence factors were explored with variance component model.【Results】 Contribution of T2DM was six times higher than age in miR-92a expression according to variance component model(sum of squared deviations were 16.676 vs 2.474).MiR-92a expression was higher in patients with T2DM than those without T2DM(F =21.86,P =0.001).The rate of multivessel coronary artery lesions was higher in patients of SAP with T2DM than those without T2DM(73.3% vs 51.2%,P =0.035).【Conclusion】 MiR-92a is up-regulation in patients with SAP complicated with T2DM.The rate of multivessel coronary artery lesions is higher in patients with T2DM than those without T2DM.It implies miR-92a could be used as blood biomarker to prevent SAP complicated T2DM development.
