血清C反应蛋白测定在儿童肺炎中的应用价值

目的探讨C反应蛋白在小儿细菌性肺炎、支原体肺炎和病毒性肺炎诊断中的应用价值。方法采用白细胞（WBC）计数和全自动透射比浊法测定200例感染性肺炎患者和180名健康者C反应蛋白（CRP）水平,并根据结果进行分组统计。结果在肺炎急性期时,112例细菌性肺炎患者中（A组）C反应蛋白阳性率为96.4%,WBC增高72例,阳性率64.3%;58例支原体肺炎患儿中（B组）阳性率为67.0%,WBC增高28例,阳性率48.2%;30例病毒性肺炎患儿（C组）中阳性率为13.3%,WBC增高2例,阳性率6.7%;180名健康对照组（D组）阳性率8.3%,WBC增高2例,阳性率1.1%。A组、B组C反应蛋白的阳性率差异具有统计学意义（P〈0.05）,均高于C组、对照组（P〈0.05）;而C组与对照组C反应蛋白阳性率比较差异无统计学意义（P〉0.05）;A组白细胞计数及分类阳性率较B、C组明显增高,差异有统计学意义（P〈0.05）;A组血清CRP阳性率较白细胞计数及分类明显增高,差异有统计学意义（P〈0.05）。结论血清C反应蛋白可作为儿童肺炎的辅助鉴别指标,其升高程度的高低对细菌性肺炎、支原体肺炎及病毒性肺炎的鉴别有临床意义,与WBC计数相结合在肺炎病原体的诊断上有较好的辅助作用,可减少误诊。

Application Value of Test on Blood Serum C Reaction Protein in the Diagnosis of Children Pneumonia

Objective To discussApplication value of test on Blood serum C reaction protein in the diagnosis of childrenbacteroidalpneumonia,mycoplasma pneumonia,viral pneumonia.Methods To test theBlood serumC reaction protein of 200 infectiouspneumonia patients and 180 healthy person through counting white blood cell and automatic transmission nephelometer and to make statistics according to the results.Results In the acute period of pneumonia,among 112 patients who have bacteroidalpneumonia（A group）,96% of C reaction protein are positive,the white blood cell of 72 patients are increasing and 64.3% of them are positive.Among 58 patients who have mycoplasma pneumonia（B group）,67.0% of C reaction protein are positive,the white blood cell of 28 patients are increasing and 48.2% of them are positive.Among 30 patients who have viral pneumonia（C group）,13.3%of C reaction protein are positive, the white blood cell of 2 patients are increasing and 6.7% of them are positive;Among 180 healthy person（D contrast group）,8.3%of C reaction protein are positive,the white blood cell of 2 patients are increasing and 1.1% of them are positive;The difference of C reaction protein between A group and B group is significant on statistics（P0.05） and either of them is bigger than C group and contrast group（P0.05）;While the difference of C reaction protein between C group and contrast group is not significant on statistics（P0.05）;the count of white blood cell and positive probability of A group are increasing obviously and is significant on statistics（P0.05）;contrast with the count of white blood cell,the positive probability of Blood serum C reaction protein in A group is increasing obviously and is significant on statistics（P0.05）.Conclusion Blood serum Creaction protein can be used as assistant guideline on the diagnosis of children pneumonia,which increasing degree is significant on the diagnosis of children bacteroidalpneumonia,mycoplasma pneumonia,viral pneumonia.And to combine with count of white blood cell and help each other can reduce wrong diagnosis.
