经尿道电切术治疗腺性膀胱炎46例报告

目的总结腺性膀胱炎发病特点,提高其诊治水平.方法腺性膀胱炎患者46例,膀胱镜检表现为滤泡样或绒毛样26例,乳头瘤样11例,慢性炎症5例,无明显改变4例.经尿道电切术后用羟基喜树碱（每次20mg）膀胱灌注,治疗6～24个月,方案为每周1次共8次,其后每月1次共10次,第2年每3个月1次.定期膀胱镜检查及组织学观察.结果随访7～48个月,平均29个月.40例痊愈,5例复发,1例无变化.结论经尿道电切或电灼,术后羟基喜树碱灌注治疗腺性膀胱炎,有助于膀胱上皮的组织学转归.

Transurethral electronresection in the treatment of cystitis glandularis:Report of 46 cases

Objective To present the clinical features of cystitis glandularis and sum up the experience on its diagnosis and treatment.Methods A total of 46 cases of cystitis glandularis underwent cystoscopy.The results showed follicular or villous changes in 26 cases,papillomatous in 11,chronic inflammation in five and no obvious change in four.Transurethral electric resection were performed.After surgery all the cases received bladder irrigation of drug.During the follow-up cystoscopy was periodically performed and the biopsy samples were pathologically examined with all the cases.Results The mean follow-up was 29 months(range,seven to 48 months).Of the 46 cases,40 fully recovered with transitional cell surface covering the bladder musoca gradually;four had relapse;and one developed canceration(adenocarcionoma).Conclusions Transurethral resection is the essential therapy for cystitis glandularis.After surgery the bladder irrigation can be helpful for urothelium recover in histology.
