社区卫生服务中心门诊病人抗菌药物使用情况的报告分析

目的通过调查上海市一家社区卫生服务中心门诊病人抗菌药物的使用情况,为卫生行政部门提供有价值的数据,为促进抗菌药物合理使用制定更有效措施提供建议。方法收集社区卫生服务中心2012年1～6月的门诊处方,对每月总处方数及金额,抗菌药物处方数及金额进行分类统计;对月销售量居前3位的抗菌药物处方药费比及联合用药的情况予以报告,分析其中存在的问题。结果在2012年1～6月中,月平均抗菌药物处方占总数的13．1％,药费占总额的6．9％;抗菌药物销量排名前1、2、3位分别是头孢替安、青霉素、阿奇霉素,都为注射制剂。抗菌药物二药联合较常见,占抗菌药物处方的3．05％。结论头孢替安销量长时间排名第一,考虑其药效学及药动学不具有优势,不良反应较多,临床减少使用为好。青霉素销量排名在第二,可予以鼓励。

Outpatient Antibacterial-usage Analysis in Community Health Service Center

Objective To investigate the outpatient antibacterial-usage in a community health service center in Shanghai, so as to provide valuable information for hygiene bureau and take more effective measures for rational use of antibaeterials. Methods The rates of antibacterials prescriptions and the cost of antibacterials in outpatients from Jan to June 2012 in a community health service centre, Shanghai, China, were analyzed. The top 3 antibacterials prescribed and the prescriptions of antibacterials in combining use were reported, and the existing problems were analyzed. Results The average rate of antibacterial prescriptions was 13.1%, accounting for 6.9% of total drug costs. The top 3 antibacterials prescribed were all for injection： cefotiam, penicillin and azithromycin. The rate of antibacterial combination is 3.05% which is relatively high. Cefotiam ranked No. 1 in all the antibacterial sold, similar to other reports in China. Conclusion Cefotiam keeps first in sold antibacterials. Considering its poor PK and PD, and its large adverse drug reactions, use of cefotiam should be reduced. Penicillin keeps the second in sold antibacterials, whose prescription should be encouraged.
