肝病患者血清前白蛋白与白蛋白检测的临床价值

目的探讨血清前白蛋白（PA）在各种肝病中的变化及临床意义，比较前白蛋白与白蛋白（ALB）在判断病情上的敏感性。方法对217例各种肝病患者和40例正常人进行血清总蛋白（TP）、白蛋白、前白蛋白的测定，并比较前白蛋白和白蛋白、总蛋白在变化程度和敏感性上的区别。结果前白蛋白、白蛋白、总蛋白三项指标在肝硬化组、肝癌组均低于正常对照组（P〈0．05或P〈0．001），前白蛋白在HBsAg阳性携带者组、急性肝炎组均有显著降低（P〈0．05），而白蛋白、总蛋白的变化则没有显著性差异（P〉0．05）。结论血清前白蛋白较其他肝功能检测项目更灵敏地反映肝功能损害；有助于判断肝病的病情及预后。

Clinical significance of detection of blood prealbumin albumin and albumin in hepatopathy patients.

Objective To observe the changes and significance of serum prealbumin(PA) in evaluation of hep0atic disease. Methods The levels of prealbumin,albumin(ALB),total protein(TP) in 217 patients suffering from hepatic disease(experimental groups which was divided into five subgroups:(A) HbsAg carriers,(B) acute hepatic,(C) chronic Hepatic,(D) hepatic cirrhosis,and(E) hepatoma) and 40 healthy persons(control group),were determined and analyzed. Results All of three biochemical parameters were obviouly lower in the subgroups of hepatocirrhosis and hepatoma(P<0.05 or <0.01).There were significant difference in level of serum PA in subgroup A and B,but not ALB and TP.There were also significant differrences in level of serum PA before and after treatment. Conclusion PA is a more specific and sensitive parameter in evaluation of hepatic function than the others,meanwhile it is helpful for prognosis.
