单侧完全性唇腭裂快速扩弓的断层研究

摘    要:目的研究单侧完全性唇腭裂(UCLP)患者快速扩弓过程中鼻上颌复合体各层的变化规律。方法对4例恒牙期的UCLP患者(平均年龄13岁)进行快速扩弓,扩弓前后对鼻上颌复合体CT扫描,应用计算机图像分析系统对CT图像进行裂隙大小和组织密度研究。结果裂隙中央面积扩大大于鼻底和口腭部,各层灰度梯度在扩弓后都有所降低。结论单侧完全性唇腭裂快速扩弓后,扩大最显著的位置在裂隙中央,扩弓后没有骨质的沉积和软组织的变化。

Abstract:Objective To study the effect of rapid maxillary expansion on the skeletal nasomaxillary structure. Methods Four UCLP patients in the period of permanent teeth (mean age: 13 years) were treated with rapid maxillary expansion. Computerized tomography was taken in pretreatment and posttreatment periods to evaluate the changes of various levels of the structure. The scanned images of each level were processed by the computer image analysis system. Results Wider splitting was exhibited at the fissure region than that of the nasal base and oromaxillaries, while the density of tissues at each level was decreased. Conclusion Rapid maxillary expansion on UCLP patient will achieve maximum expansion at the center of fissure of the maxillary bone. No mineralization of the mid-palate structure and soft tissue changes occur after treatment.
