空心加压螺纹钉固定加股方肌蒂骨瓣移植治疗青壮年股骨颈骨折

目的 探讨青壮年股骨颈骨折的治疗方法。方法 本组48例平均年龄33．5岁，其中头下型骨折19例，经颈型骨折18例，基底型骨折11例，均用3根空心螺钉固定，同时应用股方肌蒂骨瓣移植，术后制动12周，平均随访26个月。结果 4例复位不良，3例骨折不愈合，9例发生股骨头坏死。结论 带肌蒂骨瓣移植和改善局部血运，良好复位，早制动，晚负重可降低骨折不愈合和股骨头坏死率；骨折的严重的程度是影响骨折愈合的关键因素。

Use the muscle-bone flap of quadratus treatment of femoral meck fracture in the youths and the middle- age

Objective To observe the treatment of femoral neck fracture in the youths and the middle-ages.Methods There were 48 cases with an average age of 33.5 years in the group.19 were with fracture under the head,18 cases were at the neck and 11 cases at the bottom of head.All of the cases were treated with three annulated.Screw and united quadrate muscle-bone flap patients were immobilizer,for 12 weeks after operation and were followed up for 26 months.Results 4 cases were poor reduction,3 cases were nonunion,9 cases had suffered from femoral head necrosis in all cases.Conclusions Using muscle-bone flap to treat fresh femoral neck fracture early,with anatomical reduction and rigid fixation can efficiently improvedood supplement.Better fracture reduction,earlier activities and later weight carrying will decrease the rate of the femoral head necrosis.Severity of the fracture directly influences the prognosis.
