肥厚型心肌病左室舒张功能及影响因素的研究

目的评价肥厚型心肌病左室舒张功能受损程度及其可能影响因素.方法使用经胸多普勒超声心动图检测56例肥厚型心肌病患者二尖瓣及肺静脉口血流频谱来评价其舒张功能,探讨左室肥厚程度、左室流出道梗阻等因素对舒张功能损害程度有无影响.结果95%肥厚型心肌病患者存在左室舒张功能受损,其中轻、中、重度损害分别占59%、27%、9%,舒张功能损害程度同左室肥厚程度等因素均无关.结论绝大多数肥厚型心肌病患者存在不同程度的左室舒张功能受损,经胸超声联合检测二尖瓣及肺静脉口血流频谱可准确有效地评价舒张功能,肥厚型心肌病左室舒张功能受损同心肌肥厚程度等因素无关.

STUDY OF LEFT VENTRICULAR DIASTOLIC FUNCTION AND ITS POSSIBLE INFLUENCING FACTORS ON PATIENTS WITH HYPERTROPHIC CARDIOMYOPATHY

Objective To evaluate the extent of left ventricular (LV) diastolic dysfunction and its possible influencing factors on patients with hypertrophic cardiomyopathy(HCM). Methods LV function in 56 patients with HCM were assessed by mitral and pulmonary venous flows evaluated by transtho-racic Doppler echocardiography.The relationship between some factors including the extent of LV hypertrophy,LV outflow obstruction, age and sex and the extent of LV diastolic dysfunction was assessed by means of multivariate statistical analysis. Results LV diastolic dysfunction was found in 53(95%) of the 56 patients with HCM, patient with mild,moderate and severe dysfunction occupied 59% ,27% and 9% respectively. There were no correlation between extent of LV hypertrophy, LV outflow obstruction, age, sex and the extent of LV diastolic dysfunction. Conclusion The diverse extents of LV diastolic dysfunction were found in most patients with HCM; Combining mitral flow with pulmonary venous flow detected by transthdracic doppler echocardiography can assess LV diastolic function accurately and effectively; LV diastolic dysfunction in patients with HCM are independent of the extent of LV hypertrophy.
