腓肠神经营养血管皮瓣的基础研究与临床应用

目的：改进腓肠神经营养血管皮瓣的手术方法。方法：新鲜成人小腿标本20侧，对腓动脉及其分支、腓肠神经营养血管进行解剖观测；临床应用腓肠神经营养血管皮瓣治疗足背、外踝、足跟创面9例。结果：研究发现腓动脉神经营养皮支(腓动脉肌间隙皮支)以节段动脉的形式分布于腓肠神经及周围皮肤：据此设计了寻找腓动脉神经营养皮支的皮瓣游离方法；临床应用9例全部成活。结论：以寻找腓动脉神经营养皮支为特点的皮瓣游离方法使腓肠神经营养血管皮瓣手术安全、有效。

BASIC STUDY AND CLINICAL APPLICATION OF SURAL NEUROVASCULAR FLAP

Objective: To improve the surgical methods of sural neurovascular flap. Methods: 20 sides of the adult fresh cadaveric leg were chosen to make an anatomic measure. The sural neurovascular flap was used in 9 cases for foot dorsum defect and lateral malleolus and heel soft tissue defect. Results: The intermuscular cutaneous perforating branchs of peroneal artery were distributed to sural nerve and its adjacent skin in the style of segment. The surgical method of flap was designed depending on looking for the intermuscular cutaneous perforating branch. 9 cases survived completely in clinic. Conclusion: It is convenient, safe and effective to incise the flap by looking for intermuscular cutaneous perforating branch for the application of sural neurovascular flap in clinic.;
