复合性臂丛神经根撕脱伤诊治体会

摘    要:目的:讨论复合性臂丛神经根撕脱伤的诊治方法。方法:12例复合性臂丛神经根撕脱伤者行颈脊髓造影CT扫描。4例上中干型者行尺神经束移位重建屈肘功能,8例全臂丛型者行健侧颈7切除移位术重建屈肘功能。结果:臂丛神经撕脱伤性脊膜囊肿,上中干型者行尺神经移位术效果好,而全臂丛型者健侧颈7移位术效果不太满意。结论:CTM为复合性臂丛神经根撕脱者的可行检查方法,本病的修复以神经移位术为主,其方法有待完善。

Abstract:Objective To discuss the imaging diagnosis and treatment of complex brachial plexus root avalsion(CBPRA). Methods 12 cases of CBPRA were examinated by CT myelography. The function of elbow joint were restored by using fascicle nerve transfer from ulnar nerve to 4 cases with upper and middle trunks injurys, and from contralateral CT to 8 cases with all trunks injuries. Results Accuracy of diagnosic CBPRA for CTM is 100%. The main findings was formation of traumatic meningeal cyst. The functions of elbow joint restored by using fascicle nerve transfer from ulnar nerve treated upper and middle trunk injuries achieved good result, and from contralateral C 7 treated all trunk injuries not be very effetive. Conclusion CT myelography is a desriable method of examination to CBPRA. The repairment is nerve transfer, but it's method still needs introduetion and modification.
