碘油乳剂联合高强度聚焦超声损伤正常兔肝脏组织的实验研究

摘    要:
目的:探讨HL-2联合HIFU损伤正常兔肝组织的效果及靶区灰阶表现特点.方法:实验组输入HL-2后1h以一定参数行肝组织HIFU损伤,记录B超灰度变化值及治疗时间;对照以saline代替HL-2,余步骤同实验组.实验后24h解剖,测量凝固性坏死组织大小、计算EEF并行组织病理检查.结果:两组靶区灰阶表现形式相似;损伤组织大体观及病理检查两组均为典型的凝固性坏死,实验组EEF明显小于对照组(P<0.001).结论:HL-2联合HIFU能导致靶区组织发生凝固性坏死,并有增强正常兔肝脏组织HIFU能量沉积的作用,在HL-2存在的情况下,灰阶超声对靶区凝固性坏死组织的实时监控作用有待进一步研究.

Abstract:
Objective:To explore the effects of ablation of normal rabbit liver tissue by combination of high intensity focused ultrasound(HIFU) and iodized oil emulsion(HIFU liposome 2,HL-2) and the grey scale changes in target region.Methods:Rabbit liver was ablated with HIFU under certain parameters one hour after HL-2 was injected intravenously in the experimental group.The grey scale in target region and the exposure time were recorded.All processes in control group were the same as those in the experimental group only HL-2 was replaced by saline.Results:The changes of grey scale in the target region were similar in two groups.Typical coagulation necrosis happened both in two groups under gross and histological examination.The energy efficiency factor (EEF)in the experimental group was less than that in the control group(P<0.001).Conclusion:The combination of HL-2 and HIFU can cause coagulation necrosis in the target tissue of rabbit liver,and HL-2 have the function of enhancing HIFU energy deposition in the target tissue of rabbit liver.Whether the grey scale of ultrasound can be used for monitoring the necrotic tissue when HL-2 existed should be further explored.
