D-二聚体含量测定在慢性硬膜下血肿引流管拔管时机选择中的作用

目的：观察硬膜下血肿液D-二聚体含量测定对指导血肿腔留置引流管拔管的作用。方法选取2009年6月~2013年6月在我科诊断为慢性硬膜下血肿的患者160例，分为对照组与观察组各80例。所有患者均在血肿液D-二聚体含量正常时，行颅骨钻孔血肿腔留置导管引流术。比较随访3个月后的两组患者的并发症情况以及死亡率情况。结果术后随访3个月，观察组患者的术后并发症发生率及死亡率均优于对照组（P〈0.05）。结论选择血肿液D-二聚体含量正常时进行血肿腔留置引流管拔管，有助于此类患者的术后恢复，并可以有效地降低并发症发生率和死亡率。

Hematoma fluid D-2 dimer quantitative guidance of chronic subdural hematoma cavity drainage tube indwelling time to extubation

Objective To observe the effection of D dimer ditermination insubdural hematoma to guide indwelling drainage tube extubation in hematoma cavity. Methods We choose 160 pateints who were diagnosed as chronic subdu-ral hematoma and then were randomly divided into control group and observation group, including 80 cases in each group. Indications of the patients for surgery were clearly, in this context, burr holes hematoma cavity drainage.The pateints, we took drainage tube extubation measures in control group, and thus the hematoma fluid samples were re-served and the content of D dimmer was measured in the right time.The pateints in observation group were processed for hematoma cavity drainage tube extubation according to actual situation of patients and referenced the content of D-dimer when the results were normol （Gold standard immunoassay less than 0.3mg/L）. Also, we compared the complaca-tion rate and the motality rate between the two groups. Results Pateints were followed up for three months, the compla-cation rate and the motality rate in obvation group are better than that in control group （P〈0.05）. Conclusion Measur-ing the content of D-dimer and choosing the D-dimer content is helpful for the pateints of chronic subdural hematoma who were under hematoma cavity drainage tube, which could reduce the morbidity and mortality effectively.
