糖尿病早期肾损害对左室重塑的影响

目的：探讨糖尿病早期肾功能损害对左室重塑的影响。方法：124例糖尿病患者根据尿微量白蛋白水平分别分为肾功能正常组（n=68）和早期肾功能损害组（n=56），比较左室结构的变化和左室肥厚的发生率。结果：早期肾功能损害组的左室后壁厚度（LVPWT）、左室质量（LVM）和左室质量指数（LVMI）较肾功能正常组明显增加，其数值分别为（9．0±1．2）mm：（8．3±1．0）mm、（159±2）g：（11±5）g和（43±2）：（30±1），P均〈0．05。早期肾功损害组左室肥厚（LVH）的发生率为21．9％，显著高于肾功能正常组的（2．3％），P〈0．05。结论：糖尿病早期肾功能损害对左室重构有显著影响，对糖尿病患者积极预防早期肾功能损害能减少左室重构的发生。

Mild renal dysfunction and its association with left ventricular remodeling in patients with diabetes

Objective： To investigate the influence of mild renal dysfunction on left ventricular remodeling in patients with diabetes. Methods; A total of 124 patients with type 2 diabetes mellitus （T2DM） were divided into to normal renal function group （n=68） and mild renal dysfunction group according level of urinary albumin excretion （UAE） （n= 56）. The difference on left ventricular structure and the prevalence of left ventricular hypertrophy （LVH） were compared between two groups. Results： The value of LVPWT, LVM, LVMI in mild renal dysfunction group significantly increased compared with those of normal renal function group （9.0 ±1.2） mm ： （8.3 ±1.0） mm, （159 ±2） g ; （111±5） g and （43 ± 2） ： （30 ± 1）, P〈0.05 all]. The prevalence of LVH in mild renal dysfunction group was 21.9 %, which was higher than that of normal group （P〈0.05）. Conclusion; Mild renal dysfunction is one of important factors of left ventricular remodeling in patients with diabetes. If mild renal dysfunction patients with diabetes were early intervened, then ventrieular remodeling can be decreased.
