颅底炎性假瘤的诊断和治疗——附2例报道

目的探讨颅底炎性假瘤的诊断和治疗方法。方法报道2004年和2006年在我科诊治的2例颅底炎性假瘤,并结合文献对该病的临床表现、病理学特点、影像学特征、治疗方法及疗效进行总结分析。结果颅底炎性假瘤侵袭性的临床表现易误诊为恶性肿瘤,在病理学上无特异性的表现,在影像学上表现为T2加权低信号,治疗效果不佳,容易复发。结论T2加权低信号是颅底炎性假瘤的影像学特征,颅底炎性假瘤的治疗应根据自身特点,进一步优化治疗方案。

Diagnosis and treatment of inflammatory pseudotumor of skull base (report of two cases)

Objective To discuss the diagnosis and treatment of inflammatory pseudotumor of the skull base. Methods Clinical data of two patients with inflammatory pseudotumor of the skull base treated in Xiangya hospital from 2004 to 2006 were studied retrospectively. The clinical manifestations, pathological and radiological characteristics, treatment and therapeutic effect were summarized and analyzed combined with literature review. Results The inflammatory pseudotumor of the skull base showed an aggressive clinical behavior, which made it be misdiagnosed as malignancy. There was no specific pathological characteristic. The soft-tissue lesions were hypointensive on MRI T2-weighted images. The disease had poor response to steroid treatment or low-dose radiotherapy, and had high recurrence rate. Conclusion Hypointensiveness on T2-weighted images of MRI is the character of inflammatory pseudotumor of the skull base. The treatment should be individualized, and the therapeutic strategy should be optimized furthermore.
