人工特制假体在下肢骨肿瘤保肢治疗中的应用

目的探讨应用人工特制假体在下肢骨肿瘤保肢治疗中的方式和疗效。方法35例下肢骨肿瘤患者,23例行定制带股骨(或胫骨)铰链式膝关节,8例行加长柄人工股骨头置换,4例行加长柄全髋置换。结果治疗后随访9～47个月,平均22个月,生存的30例按Enneking肢体肌肉骨骼肿瘤外科治疗重建术后功能评估标准进行评估。4～5分26例,3分3例,1分1例。结论采用合理的肿瘤边缘完整切除手术,选择特制人工假体并正确安装,辅以综合治疗,能有效提高下肢骨肿瘤保肢率及最大限度保留肢体的功能。

The application of prosthesis replacement in limb salvage treatment of bone tumors of lower limb

Objective To evaluate the positive application of joint prosthesis in limb salvage treatment of bone tumors of lower limb. Methods 35 patients with bone tumors of lower limb were treated with en bloc resection and prosthetic replaced. The type of the prostheses were as follows: 23 custom-made hinged knee prostheses with femoral or tibial component, 8 long stem hip prosthesis, 4 hip prosthesis. Results All cases had followed-up for average 22 months ( 9 months to 47 months) and 30 alive patients were evaluated in regard to function using Enneking system. 18 cases had excellent results, 8 cases good, 2 cases fair and one case poor. Conclusions The limb salvage treatment in cases of lower limbs tumor can be attained perfectly by resecting tumor reasonably with adequate margin and replacing moderate joint prosthesis and then comprehensive treatment.
