Abstract:
Objective To observe the effect of antibiotic de-escalation therapy on neonatal infectious pneumonia.Methods A total of 100 cases of neonatal infectious pneumonia were divided into control group and observation group according to random numerical table,with 50 cases in each group.The control group received empirical antibiotic treatment,and the observation group received sensitive antibiotics according to sputum culture results after 5 d of treatment.The clinical symptom disappearance time,hospitalization time and related indicators[white blood cell count(WBC)and C-reactive protein(CRP)]were compared between the two groups.Results The lung rales disappearance time,stable breathing time,body temperature recovery time and hospitalization time of the observation group were(72.5±8.1),(45.6±7.6),(25.1±6.4)and(162.4±15.9)h,which were all shorter than(135.6±10.2),(82.7±9.8),(42.9±10.7)and(251.7±25.6)h of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in WBC and CRP between the two groups(P>0.05).After treatment,the WBC and CRP of the two groups were lower than those before treatment,and the difference was statistically significant(P<0.05).Conclusion Antibiotic de-escalation therapy can speed up the symptom relief and shorten the hospitalization time of neonates with infectious pneumonia.There is no significant difference in WBC and CRP levels between empiric antibiotic therapy and antibiotic de-escalation therapy for 7 d.
