使用呼吸机与重症监护病房患者下呼吸道感染的相关性分析

摘    要:
目的:分析使用呼吸机与重症监护病房患者下呼吸道感染的相关性。方法:本次实验研究将2018年1月~2019年1月在本院重症监护病房接受治疗并使用呼吸机的患者32例作为实验组,将同期本院重症监护病房收治未使用呼吸机的患者32例作为对比组,比较两组患者下呼吸道感染发生率以及导致实验组患者出现下呼吸道感染的相关危险因素。结果:实验组患者下呼吸道感染发生率明显高于对比组患者(P<0.05);实验组不同机械通气时间、基础疾病数量以及插管方式患者之间出现下呼吸道感染的概率存在显著差异(P<0.05);导致实验组患者出现下呼吸道感染的病菌种类为革兰阴性菌概率显著高于革兰阳性菌与真菌(P<0.05)。结论:使用呼吸机与重症监护病房患者下呼吸道感染的出现呈正相关,且导致其出现下呼吸道感染的危险因素包括机械通气时间、基础疾病数量以及插管方式,革兰阴性菌为主要的致病菌,应该通过针对性预防措施的制定减少下呼吸道感染的出现。

Abstract:
Objective:To analyze the correlation between the use of ventilator and lower respiratory tract infection in patients in intensive care unit.Methods:In this experimental study,32 patients who received treatment in the intensive care unit of our hospital from January 2018 to January 2019 and used a ventilator were used as the experimental group,and 32 patients who did not use the ventilator were admitted to the intensive care unit of our hospital during the same period.The case was used as a comparison group to compare the incidence of lower respiratory tract infections and the related risk factors of lower respiratory tract infections in patients in the experimental group.Results:The incidence of lower respiratory tract infection in the experimental group was significantly higher than that in the control group(P<0.05);there was a significant difference in the probability of lower respiratory tract infection among patients in the experimental group with different mechanical ventilation time,number of basic diseases,and intubation methods(P<0.05);the probability of the lower respiratory tract infection causing Gram-negative bacteria in the experimental group was significantly higher than that of Gram-positive bacteria and fungi(P<0.05).Conclusion:The use of ventilator is positively correlated with the occurrence of lower respiratory tract infections in patients in the intensive care unit,and the risk factors that lead to the occurrence of lower respiratory tract infections include the time of mechanical ventilation,the number of basic diseases and the way of intubation.Therefore,the occurrence of lower respiratory tract infections should be reduced through the development of targeted preventive measures.
