老年慢性病患者的心理状况分析

目的 :了解老年慢性病患者的心理状况 ,探讨其影响因素 ,从而提出相关对策。方法 :收集湘雅医院老年科门诊及住院的 6 0岁以上慢性病患者 ,进行全面体格检查和必要的辅助检查 ,并进行症状自评量表 (SCL -90 )调查。结果 :在该老年慢性病患者人群中有 77.33%患者有心理障碍 ,主要问题是躯体化、抑郁、人际关系、强迫、焦虑等 ;多元逐步回归分析表明影响该老年病患者人群心理健康的主要因素为经济收入、服药种类及家庭结构 ,其回归方程为 :Y心理 =136 .96 +5 .0 1X药物 - 6 .6 0X经济 - 3.4 3X家庭(P <0 .0 5 ) ;不同慢性疾病患者心理障碍发生率无显著差异 (P >0 .1)。结论 :该老年慢性病患者人群心理障碍是普遍的 ,主要受经济收入水平、躯体健康状况及家庭结构等因素的影响 ,改善老年人的躯体健康、经济收入和家庭结构以及心理疾病治疗可提高老年慢性病患者的心理状况。

MENTALITY CONDITION AND ITS AFFECTED FACTORS OF AGED PATIENTS WITH CHRONIC DISEASES

Objective:To explore the condition and affected factors of aged patients' mentality. Methods:All the subjects were aged more than 60 years and were in Geriatrics Department of Xiangya Hospital. General physical examination and essential auxiliary examination were executed. Symptom checklist 90(SCL-90)was measured. Results:The problems of mentality were occurred in 77. 33% of the patient.Multiple linear regression showed that financial condition, family structure and physical diseases were major factors that affected the patients' mentality condition. The regression equation was Y mentality=136.96+5.01X 1-6.60X 2-3.43X 3.The reduction rate of mentally condition to the patients with different chronic diseases was at the same level.Conclusion: The reduction of aged patients' mentality condition was general. The major affected factors were financial condition, physical diseases and family structure. To improve aged patients' financial condition,family structure and health can perfect their mentality condition.
