三种无痛人流麻醉的临床观察

目的对3种不同的镇痛方法应用于人工流产术的临床效果进行对比分析。方法将300例早期妊振要求行无痛人工流产的孕妇随机分为三组：A组为丙泊酚静脉注射组，B组为舒芬太尼＋丙泊酚组，C组为布托啡诺＋丙泊酚组。观察三组的静脉注射痛、宫预松弛程度、头晕、呕吐、术中出血量、术后腹痛、体动及丙泊酚用量等情况。结果B、C组宫颈松弛程度、术中体动与A组比较有差异（P〈0．05），B、C组静脉注射痛、术后腹痛及丙泊酚用量与A组比有显著性差异（P〈0．01）术后清醒时间、术后出血量无差异（P〉0．05），B组头晕发生率比A、C组比较，有显著性差异（P〈0．01）。结论布托啡诺复合丙泊酚应用于无痛人流效果满意，不良反应少，值得在基层医院推广和应用。

The clinical observation of three painless induced abortion

Objective To compare the clinical effects of three different types of anesthesia treatment on induced abortion. Methods 300 early pregnant woman 300 pregnancy patients who asked for painless induced abortions were randomly divided into three groups, one with propofol intravenously, the other with propofol and sufentanil intravenously, and the third group with propofol and butorphanol intravenously. Observe the venous pain and the degree of cervix relaxation, dizziness, vomiting, bleeding volume, postoperative pain, physical activities and dosage of propofol in deferent groups. Results There were significant different in degree of cervical relaxation and physical activities between group B and C with group A (P<0.05), and also there were quite significant difference in intravenous pain, postoperative pain and propofol dosage between group B, C and A (P< 0.01).There was no difference in the time of waking and bleeding volume among the three groups. The incidence of dizziness of group B was remarkably higher than group A and C (P<0.01).Conclusion Butorphanol combined with propofol used in induced abortion was safe and effective and fewer side effects. It is worth promoting and applying at the grass-root hospitals.
