高血压患者静息心率与左心室肥厚关系的临床探讨

目的探讨高血压患者静息心率(RHR)与左心室室壁肥厚的相关性。方法高血压病患者220例,按RHR水平分为3个亚组,RHR1亚组:<70次/min(34例);RHR2亚组:70～80次/min(92例),RHR3亚组:≥80次/min(94例)。健康对照组120例为健康体检者,心脏功能正常。测量各组患者室间隔厚度(IVSD)和左室后壁厚度(LVPWD)。结果(1)高血压组RHR(80±12)次/min显著高于对照组RHR(70±8)次/min(P<0.05)。(2)RHR3亚组的IVSD、LVPWD显著高于RHR1亚组和RHR2亚组,RHR2亚组的IVSD、LVPWD显著高于RHR1亚组(P<0.05)。(3)高血压患者的RHR与IVSD、LVPWD呈正相关(P<0.05)。结论(1)高血压患者RHR较健康者显著增加;(2)高血压患者随着RHR的增加,发生左心室肥厚的机会也增加。

Correlation between resting heart rate and left ventricular hypertrophy in patients with hypertension

Objective To investigate the correlation between resting heart rate（RHR） and left ventrieular hypertrophy in patients with hypertension.Methods 220 patients with hypertension were divided into three groups according to their levels of RHR： group 1 （ RHR： 〈 70 times/min, n = 34）, group 2 （ RHR： 70 - 80xtimes/min, n = 92）, group 3（RI-IR： ≥ 80 times/min, n = 94）. 120 healthy subjects were used as control group, and these subjects were also divided into three groups：group one（RHR： 〈 70 times/min, n = 53） ,group two（RHR：70 - 80 times/min, n = 40）,group three（RHR： 〉180 times/min, n = 27）,the LVPWD and IVSD were detected by ultrasound in all the patients and healthy subjects. Results （ 1 ） The RHR of hypertensive patients was significantly higher than that of the control group（80 ± 12 vs 70± 8, P 〈 0.05）. （2） The IVSD and LVPWD of group 3 were significantly higher than those of group 1 and group 2, and the IVSD and LVPWD of group 2 were sigrfifieantly higher than those of group 1 （ P 〈 0.05）. （3） The IVSD and LVPWD were positively correlated with RHR in hypertensive patients（ P 〈 0.05）. Conclusion （ 1 ） The RHR of hypertensive patients is significantly higher than that of healthy subjects. （2） With the increase of RHR, the opportunity, of left ventricular hypertrophy also increases obviously.
