藏、回、汉族胃癌患者hMLH1和hMSH2基因的差异性表达及意义

摘    要:
目的 检测并分析藏、回、汉三民族间胃癌患者错配修复基因hMLH1、hMSH2的表达异同及临床意义.方法 采用免疫组织化学法,检测藏、回、汉族胃癌组织标本hMLH1、hMSH2基因的表达,并取相应民族同期正常浅表性胃炎患者组织标本作对照.结果 ①汉、回、藏族胃癌组织中hMLH1阳性检出率分别为70.00%、66.67%、33.33%,在浅表性胃炎中分别为90.00%、86.67%、93.33%.胃癌组织中的阳性表达率低于浅表性胃炎组织.其中,藏族hMLH1在胃癌中的阳性表达率显著低于汉、回族,回、汉族间hMLH1的胃癌表达相比无统计学意义.浅表性胃炎组中三族相比无统计学意义.②汉、回、藏族胃癌组织中hMSH2阳性检出率分别为96.67%、90.00%、93.33%,在浅表性胃炎组织中分别为43.33%、50.00%、50.00%.在胃癌组织的阳性表达率为93.33%,高于浅表性胃炎组(47.78%).汉、回、藏族胃癌组织hMSH2的表达均高于浅表性胃炎组织,而在胃癌及浅表性胃炎组中三族相比均无统计学意义.结论 hMLH1在藏族胃癌组织呈显著性低表达,可能与藏族较回、汉两民族更易发生胃癌有关.hMSH2在藏、回、汉三民族胃癌组织中呈普遍性的高表达,可否将其作为普遍的肿瘤标记物仍需进一步研究.

Abstract:
Objective To detect and investigate the similarity and difference of the expression of mismatch repair genes hMLH1 and hMSH2 in Tibetan,Hui and Han patients with gastric cancer and its significance.Methods Immunohistochemistry was employed to detect hMSH2 and hMLH1 genes expression in gastric cancer compared that of in normal superficial gastritis.Results ① The positive rates of hMLH1 expression in cancer among Tibetan,Hui and Han patients were 70.00%、66.67%、33.33%,respectively,while 90.00%、86.67%、93.33%,respectively in gastritis.The former were lower than the latter.Meanwhile,the positive rate in the cancer of Tibetan patients was significantly higher than those in Hui and Han patients,while the positive rates between Hui and Han patients showed not significant difference.There were no significant difference of positive rates in gastritis.②The positive rates of hMSH2 expression in gastric cancer of Tibetan,Hui and Han patients were 96.67%、90.00%、93.33%,respectively,while 43.33%、50.00%、50.00%,respectively,in gastritis.The positive rate 93.33% in cancer is higher than that 47.78% in gastritis.The positive rates of hMSH2 expression in cancer of three nationalities were all higher than those in gastritis.While the positive rates in gastritis and cancer of three nationalities were not significant difference.Conclusions The significant low expression of hMLH1 gene in Tibetan patients with gastric cancer supports the theory of the susceptibility to gastric cancer for Tibetan nationality.The positive rates of hMSH2 expression in three nationalities were not significant difference and it as a common tumor marker needs to be studied further.
